The successful candidate will have/be:
- Experience within the care industry is essential
- Meeting clients approximately 3 days a week, up to an hour and half from home
- Multitask
- Manage diary
- Supporting client and carer
- Empathetic
- Building a good rapport
- Support for client’s family
As a Care Co-Ordinator, this role will be Home-Based after induction. You will be going to clients home and building a care assessment/plan to meet their needs, preferences and hobbies.
Duties will include, but are not limited to:
- Previous Regional Consultant/Regional Manager experience within the Healthcare/Homecare industry required.
- Level 4 qualification or above would be advantageous but not essential, experience in Health and Social Care important.
- Clinical skills, previous assessment experience, knowledge and understanding of CQC guidelines, understanding of auditing processes.
- Demonstrated ability to:
- Make decisions using professional expertise within a structured framework and using existing systems and policies.
- Work effectively, both independently and as part of a team.
- Effectively prioritise workloads and time management.
- Understand the importance of completing detailed assessment, involving the client, family / representatives, and other professionals in every stage of the process.
- Provide support to the carers, with the necessary information to enable them to deliver a high-quality level of service to all clients.
- Current, clean UK drivers’ licence and access to your own vehicle.
Salary: Circa £29,000
Hours: Monday – Friday 9am – 5.30pm
Location: Tonbridge
