This is an exciting opportunity to work as part of a service delivering re-ablement in patient »s own homes as part of the Carrick Home First service which is a collaborative approach between community, acute and social care providers.

Working for Cornwall NHS Foundation Trust the successful candidate will demonstrate a caring approach and a desire to work with people to support them to identify and achieve their goal. An ability to motivate individuals to support their recovery and to maximise their independence and improve the quality of people lives is essential.

You will need to demonstrate good communication skills and a positive approach to people who may have communication difficulties or who have challenging health needs.

This role involves frequently dealing with confidential and sensitive information along with performance monitoring which entails validating and raising any data quality issues for key provider performance reports including Referral to Treatment validation, activity and waiting list reporting. Bespoke training to support your development in this role will be provided.

You will be required to work a rota that covers 7 days a week from 08:00 to 20:00 hours. Flexibility within this rota, and geographical location, will be important.

Main duties of the job

  • The main purpose for this post is to ensure that opportunities for patients’ reablement are optimised within a hospital and community setting.
  • The post holder will be rostered with other Generic Support Workers to provide a 7 day a week service to inpatients and or community patients requiring reablement either during or after a hospital admission.
  • The Reablement workers within each locality will form a part of an Integrated Care Pathway for frail patients that involve a range of services.
  • This post holder will be expected to work with patients on the ward, in therapy areas and in the community, i.e. Residential homes and in patients own homes.
