Under the supervision of the Director Operations, the Practice Coach supports the team in the implementation and sustainment of the AdvantAge Model. Providence Advantage is a new Senior focused value based primary care model. The Practice Coach assists with the daily management and sustainment of the new care delivery model by using the knowledge, skills and tools of team-based care to facilitate practice transformation across the system. The Practice coach will be responsible for monitoring performance and use problem solving techniques to assist in removing barriers and keeping providers, management and staff engaged.

This position must be located out of Providence »s Irvine or Anaheim locations and the role will be conducted up to 75% of time in clinic locations. Ability to travel to WA clinics is a requirement of the role.

Required qualifications:

  • Bachelor »s Degree in Computer Science, Business Administration, Health Care Administration, or related field or High School Diploma plus 4 years relevant experience and appropriate certifications
  • National Provider BLS – American Heart Association upon hire
  • 2 years experience in health care operations
  • Experience working effectively with senior executives, management, staff, physicians, contract personnel and vendors

Preferred qualifications:

  • 5 years experience in a health care or managed care system
  • Project management experience in an ambulatory care environment
  • Experience with Six Sigma quality improvement processes and tools
  • Experience with Lean Production/Manufacturing processes and tools
