Job description

Recruiting for an Enablement Worker to join our Mental Health service in Lambeth.

£25,000.00 per annum, working 32 hours per week, plus 25 days Annual Leave, Pension schemes, Staff discounts, healthcare and more.

Job Overview:

Lambeth Independent Living and Medication Support Service is a community floating support service for adults with a primary mental health need and a range of dual needs. We support adults living in independent accommodation across the borough of Lambeth.

You will provide support to the service through building partnerships with other organisations and agencies. They will develop and enhance community networks and engagement within Lambeth, including accessing local grants and resources to assist with resettlement for our customers.

Key Responsibilities:

  • Ensure co-production is embedded within the culture of the service as a ‘way of life’
  • Support team of Specialist Support Workers in helping customers develop networks and skills for independence, as appropriate to the needs, skills, abilities, and interests of the customer
  • Maintain up-to-date information-sharing protocols/Service Level Agreements (SLAs)
  • Organise shadow days, joint training/meetings to upskill staff and build relationships
  • Invite delivery of in-reach sessions at drop-in groups
  • Develop relationships with community partners
  • Supervise medication for customers and follow all protocols in line with our Medication Policy and Procedure.
  • Create partnerships to meet emerging needs of customers, e.g., assistive technology for physical health needs
  • Support the team’s rapid response to crisis/relapse, preventing hospital admission
  • Effective risk management during transition, e.g., joint visits to attend housing/medical appointments
  • Increase staff understanding of clinical language, roles, and practice, through specialist training
  • Liaising with staff in other Lambeth mental health services and community agencies to develop customer support networks/opportunities
  • Network with the local community, organisations, to further develop co-produced peer support and peer-led sessions with customers
  • Carry out support duties to enable customers to integrate into the community including e.g., accompanying the customer to attend benefit agency appointments and meetings, go shopping, to enquire at colleges about possible courses, to visit various public amenities.
  • Accessing and applying for grants/donations for customers
  • Report any observations relating to customers welfare in line with Look Ahead’s Safeguarding policies.
  • Adhere to all the policies and procedures of Look Ahead plus those specific to the service including any statutory requirements.
  • Maintain records as required at the service.
  • Participate in team meetings/reviews and the general development of the service.
  • Undertake any other duties consistent with the grade and nature of the post as may be reasonably assigned.

Skills, Knowledge and Experience
Desirable:

  • NVQ Level 2 or equivalent or experience within charity/social care sector
